PERIODONTAL EXPOSURE SURGERY OF THE IMPLEMENTED MAXILLARY CANINE IN THE ORTHODONTICS DEPARTMENT

Summary :

Between January 2010 and December 2022, in the context of a private periodontic practice, 1363 impacted teeth were exposed for orthodontic purposes. Maxillary canines constituted almost 75% (1012) of all teeth, while the mandibular canine group was 5.06% (69).
 
This decidedly clinical communication will address the different inclusion situations, from the simplest to the most extreme, and a surgical decision tree will be proposed.
The various periodontal exposure surgeries, which help the orthodontist to position the impacted tooth with all periodontal tissues and the best aesthetic integration, will be illustrated.

PREVENTIVE TREATMENT OF CANINE IMPLEMENTATIONS AND RETENTIONS

Summary :

Maxillary canines are the second most frequently impacted teeth, with a prevalence rate of 1% to 3%. Palatal impaction is more common in Caucasians. When left untreated, impacted maxillary canines can interfere with the alignment of adjacent teeth, reduce arch length, increase the risk of follicular cyst formation and recurrent infections. They can also cause external resorption of adjacent teeth, as well as irreversible damage that may lead to tooth loss.

Through various clinical cases, we will develop the different prophylactic methods to optimize the development of permanent canines:

Is there a preventative approach to this inclusion?

o What are the first clinical signs;
o The radiological signs and appropriate views.

– What are the preventative measures and their timing?

o Transverse expansion / disjunction;
o Distalization of lateral sectors;
o Extraction of milk canines.

– Common sense therapeutic approach to inclusion:

o Surgical clearance before or after space creation;
o Torque control in areas adjacent to the inclusion;
o What to do in case of resorption

CONSIDERATIONS ON THE ROOT RESORPTION OF TEETH ADJACENT TO THE IMPLEMENTED CANINE.

Summary :

Canines are frequently affected by eruption disorders, which can impact the roots of the lateral incisors that guide their eruption. Understanding the processes of root resorption, making appropriate differential diagnoses, and choosing the right treatment approach, while considering the impact on the endodontium, is essential.
