In young patients, early periodontal management relies on a true partnership between orthodontists and periodontists. Frenula and mucosal adhesions, often overlooked, can disrupt oral hygiene, contribute to gingival recession, and compromise orthodontic stability. Their management requires collaborative planning to determine the most appropriate time for intervention and to avoid any aesthetic or functional consequences.
At the same time, the recognition of a fine periodontal phenotype in the anterior mandibular region raises the question of possible tissue reinforcement before, during or after orthodontic treatment.
In some cases, simple orthodontic correction, by eliminating the "wire syndrome," can be sufficient to stabilize the tissues and make grafting unnecessary. This integrated approach illustrates how essential interdisciplinary collaboration is for preserving tissue, ensuring long-term stability, and guiding our treatment choices. When to intervene? And at what stage? The analysis of clinical cases, supported by the literature, will provide some answers.
